It was actually based on a tyre lever. The idea is to give enormous 
cramping force.

Note that at the very beginning (and maybe more, edited out before it 
starts) the doctor strokes the baby's penis to get an erection.

Notice also that the sheet is writhing throughout. The baby is 
clearly struggling against what is being done to him. 

Halfway through, his little hand comes past. This is not supposed to 
happen. It's normally held down into the Circumstraint (TM) by velcro 
straps. 

I don't normally watch circumcision videos. I hate the whole idea.
